Advanced glycation end products (AGEs) are proteins or lipids that become glycated as a result of exposure to sugars. They can be a factor in aging and in the development or worsening of many degenerative diseases, such as diabetes, atherosclerosis, chronic kidney disease, and Alzheimer's disease. AGEs affect nearly every type of cell and molecule in the body and are thought to be one factor in aging and some age-related chronic diseases. They are also believed to play a causative role in the vascular complications of diabetes mellitus. Under certain pathologic conditions, such as oxidative stress due to hyperglycemia in patients with diabetes, and hyperlipidemia,AGE formation can be increased beyond normal levels. AGEs are now known to play a role as proinflammatory mediators in gestational diabetes as well. The animal and human evidence is that significant amounts of dietary advanced glycation end-products (dAGEs) are absorbed and that dAGEs contribute to the body's burden of AGE, and are associated with diseases such as atherosclerosis and kidney disease. In the context of cardiovascular disease, AGEs can induce crosslinking of collagen which can cause vascular stiffening and entrapment of low-density lipoprotein particles (LDL) in the artery walls. AGEs can also cause glycation of LDL which can promote its oxidation.Oxidized LDL is one of the major factors in the development of atherosclerosis.Finally, AGEs can bind to RAGE (receptor for advanced glycation end products) and cause oxidative stress as well as activation of inflammatory pathways in vascular endothelial cells. In other diseases The formation and accumulation of advanced glycation endproducts (AGEs) has been implicated in the progression of age-related diseases. AGEs have been implicated in Alzheimer's Disease, cardiovascular disease, and stroke. The mechanism by which AGEs induce damage is through a process called cross-linking that causes intracellular damage and apoptosis. They form photosensitizers in the crystalline lens, which has implications for cataract development. Kim Yale University Advanced glycation end-products are a class of natural products that form non-enzymatically on exposed protein residues in the human body. AGEs accumulate as a result of normal metabolism and aging, and significant elevations in these molecules have also been observed in the plasma of patients with chronic diseases, such as diabetes, cancer, arthritis, cardiovascular disease, and others. Our laboratory is taking an orthodox approach to studying these materials; we have initiated a synthetic program to prepare AGE- adducts on large scale and in chemically homogenous form. This talk will describe ongoing efforts along these lines, with a particular focus on exploring a class of arginine-derived AGEs. Chemical and biological insight arising from these studies will also be discussed. It is our hope that this small molecule-based strategy will serve to shed new light on the role of AGEs in both healthy and disease physiology.

Advanced Glycation Endproducts (AGEs) and the Pathophysiology of Angiopathies in T2DM

Diabetes is a disease of altered metabolism and chemistry of both carbohydrates and lipids. Normally, the absorption of carbohydrates stimulates insulin secretion and the translocation of GLUT 4 from muscle and adipose tissue's intracellular compartments to the cell's surface in order to increase cellular uptake of glucose. However, due to insulin resistance (IR) in T2DM muscle and adipose tissue are unable to bring glucose inside their cells, which causes blood glucose levels to rise (hyperglycemia). Consequently, cells that are unable to decrease the rate of glucose transported inside the cell become damaged. Specifically, this is seen in three cell types: the capillary endothelial cells of the retina, mesanglial cells in renal glomerulus, and neurons and Schwann cells of peripheral nerves; representing microvascular complications retinopathy, nephropathy, and neuropathy. However, injury to macrovascular tissue as evidenced in atherosclerosis, cardiomyopathy, and peripheral arterial disease is preceded by elevated of free fatty acids (FFAs) and low-density lipoproteins (LDLs); precursors for the formation of foam cells and fatty streaks that eventually lead to arterial insufficiency. Crucially, at the center of each of these pathologies is the formation of advanced glycation endproducts (AGEs). Advanced glycation endproducts (AGEs), or glycotoxins, are modification of proteins and lipids that become glycated and oxidized through a non-enzymatic reaction with reducing sugars (glucose, fructose, and galactose). Once formed, AGEs may bind to its receptor to disrupt normal cell function or glycate proteins in the extracellular matrix to inhibit cell growth and secretory abilities. AGEs have been demonstrated to activate inflammatory and pro-oxidant mechanisms, impair cell-signaling pathways, accumulate inside and outside the cell as well as in serum and skin, and alter cellular structure and function apart from overnutrition or genetic susceptibility.Consequently, the formation of AGEs is accelerated in diabetes because of increased availability of reducing sugars, implicating glycotoxins in the pathogenesis of vascular complications in T2DM.

Elevated levels of endogenous AGEs are directly correlated to amount of AGEs consumed in the diet, providing evidence towards their role in health and disease. Fortunately, reducing exogenous AGEs can be achieved by the way food is prepared without altering its caloric content. In fact, reduction of dietary AGEs has been demonstrated to reduce serum levels of AGEs between 30-40%, improve insulin resistance, suppress tissue injury, as well as reduce inflammatory levels of TNF-α and C-reactive protein. This new evidence suggests that the way food is prepared may be viewed as a principal risk factor in progression of T2DM diabetes. AGEs are naturally present in food. However, in the presence of heat or thermal processing the formation of dAGEs is accelerated. Dry heat, such as using a microwave, impacts AGE formation differently than broiling (225°C), grilling, boiling (100°C), roasting (177°C), searing, oven frying (230°C), and deep-frying (180°C). On the other hand, lower cooking temperatures, shorter cooking times, and cooking with moist heat, as well as the use of lemon juice or vinegar-based marinades suppress the formation of AGEs. Equally important, the foods origin, type, and cooking method individually contribute to the formation of AGEs. Animal-derived foods are generally higher in AGEs than foods without animal origin. Likewise, foods in their raw and uncooked state have fewer AGEs than their cooked counterparts. Of importance, fruit, vegetable, and whole-grains contained the lowest amount of dAGEs, even after cooking. Albeit heat or thermal processing foods is necessary for food safety and to enhance flavor, it is important to understand that the consumption of dAGEs directly contributes, in various amounts, to the endogenous pool of AGEs.

He explains that advanced glycation end-products (AGEs) can irreversibly link proteins together, and cross-linking AGEs appear to play an important role in aging.
