Home Health Value-Based Purchasing (VBP), also known as Pay for Performance, is reimbursement based on quality instead of quantity (prospective or fee basis) of care provided. The Affordable Care Act (ACA) required CMS to build a healthcare system delivering improved care, reducing cost, and resulting in healthier people and communities, thus empowering them to implement payment reform through Home Health Value Based Purchasing.https:/Medicare Value-Based Purchasing (VBP) Program is the keystone of healthcare reform for providers. The VBP Program directs the Centers for Medicare and Medicaid Services (CMS) to reward or to penalize providers on the basis of delivering higher quality healthcare services at lower cost. CMS, the largest payer in the American healthcare system, rewards and penalizes healthcare providers via citations and payments. https://www.axxess.com/value-based-purchasing Ask me a question: [email protected]
Surviving Value-Based Purchasing in Healthcare
How does your health system's quality of care measure up? With the shift toward a value-based purchasing model that rewards value, outcomes, and patient satisfaction instead of merely volume, providers need to be prepared as the country moves to implement value-driven healthcare. In a world of accountable care, quality measures, shared savings, and bundled payments, today's focus must shift to affordability in conjunction with obtaining higher value.

2017 Population Health Management - 3 Secrets To Value-Based Purchasing Success in Healthcare
Class Description 2017 Population Health Management - 3 Secrets To Value-Based Purchasing Success in Healthcare Healthcare is changing now, more than ever, and how we pay for that care is no exception. Given that healthcare spending is currently 17.8% of our nation’s GDP, the government is looking for different methods that will help reduce spending. There are several different models in place including Comprehensive Care for Joint Replacement (CJR), the newer cardiac bundle, and Bundled Payment for Care Improvement developed by CMS as a possible solution. With the new bundled payment model providers are compensated for an “episode of care”; placing the emphasis on quality of care and cost controls. But what does this new payment model mean for patients and providers? In this program, we will examine some of the types of payment models, the benefits and the drawbacks, and what potentially lies ahead. With this webinar, you will gain insight and tactics to: 4 models of care under the Bundled Payment for Care Improvement (BPCI) Initiative Comprehensive Care for Joint Replacement Model (CJR) Bundled Payment Models for Cardiac Care Bundled payment effect on coordinated care How new model reduces costs for patients, hospitals, and third party payers & creates a uniform “product” to allow for increased competition How to define episodes of care when multiple conditions interact Whether or not bundled payments cause an avoidance of high- risk patients or deter ordering necessary, but expensive, tests? How non-compliant patients impact cost What “episode of care” will be bundled next Will bundled payment model remain given uncertainty of the ACA? Will bundled payments lead to more proactive planning to reduce post- acute care costs Implementation challenges moving forward Facilitators: Greg Wahlstrom, MBA, HCM, President & Chief Executive Officer, The Healthcare Executive Stacey Klinker, MBA, HCM, Consultant, The Healthcare Executive Dr. Mandeep K. How to Win Under Home Health Value Based Purchasing
On January 1, CMS rolled out the Home Health Value-Based Purchasing (HHVBP) program as a nine-state pilot, with a focus on reducing hospitalizations. The bottom line is that if home health agencies don’t reduce the number of unplanned hospitalizations during the first 60 days of home health, then they stand to lose a substantial amount of revenue – as much as 8% by 2022. With so much at stake, home health agencies need to embrace advanced technologies that are proven to improve both the efficacy and efficiency of the care they provide. Perhaps the greatest opportunity lies with remote patient monitoring, or telemedicine, which can positively affect 15 of the 24 process and outcome measures – reduced hospitalizations included – that comprise an agency’s HHVBP score. No wonder that 89% of healthcare executives, in a recent survey, said that telemedicine will transform the U.S. health care system in the near future. Jumping into telemedicine, however, requires careful thought and planning. Considerations include the customization of available telemedicine technology, the integration of monitoring and reporting solutions into existing care processes, and more. Make a wrong turn, and it can easily drain your resources – financial, operational, and human. In this dynamic 60-minute webinar, you’ll discover how to harness telehealth to provide actionable data from patients’ homes, improve access and outcomes, decreasing costs, and maximize the financial opportunities of HHVBP. What You’ll Learn How remote patient monitoring can reduce hospital admissions by as much as 53%. The positive impact remote patient monitoring has on emergency care visits, timely and accurate patient information, patient outcomes, and HHCAHPS scores. Remote patient monitoring implementation strategies that ensure improvement in efficiency and net income. The role of the remote care manager and strategies for team integration. A best-practice remote patient monitoring implementation roadmap. About the Presenter Karen Thomas is the president of Advanced TeleHealth Solutions, a URAC-accredited health call center providing customized telemonitoring and health call center solutions. In 2004, she developed and secured funding for the first Medicaid-covered telemonitoring program within the Disease Management Department for the State of Missouri. Karen is a nationally renowned speaker, using her expertise in telehealth monitoring to educate hospitals, home health agencies, and other companies and organizations about the benefits of telemedicine solutions.
